Pathologic 3 receives unexpected news, will be released on both consoles and PC in 2025.
The game is a sequel to Pathologic 2 released in 2019, which is a remake of the original game Pathologic from 2005.
Ice-Pick Lodge, a Russian development studio, has revealed the newest installment in the Pathologic series: a sequel to Pathologic 2 from 2019 named Pathologic 3. The studio has also verified that supporters of the series who funded Pathologic 2 through its original 2014 KickStarter campaign will receive Pathologic 3 at no cost.
The disclosure was made together with a preview and a direct link to the game’s Steam page, enabling players to add it to their wishlists. While Ice-Pick Lodge has not provided a specific release date, they have assured that the next installment of Pathologic will come to PlayStation 5, Xbox Series X|S, and PC in 2025. The contents of the trailer show that players will relive the events of Pathologic 2 in Pathologic 3, but this time from the perspective of Danil Dankovsky, also known as “The Bachelor.”

Taking place in a tiny Russian town on the isolated steppes during the early 1900’s, Pathologic tells the tale of three key figures: Artemy Burakh, known as “The Haruspex”, a local healer who returns to his hometown to address a recent tragedy; Danil Dankovsky, or “The Bachelor”, a certified doctor from the city hoping to make a scientific breakthrough; and Clara, also known as “The Changeling”, a mysterious girl with the power to heal or harm with a touch, but with no knowledge of how or why she ended up in the town. There is no information yet on when players will be able to play Clara’s account, but with the announcement of Pathologic 3, it is likely that Clara’s gameplay is also being developed.
In 2005, the initial Pathologic release received praise in Russia and gathered a dedicated fanbase globally. The initial game was later succeeded by a remastered edition with enhanced visuals and a better English translation. Both iterations of the game enable players to finish the full story of each main character, with each playthrough uncovering additional details about the mysterious illness that is quickly claiming the lives of the town’s residents.
Every main character needs to discover a remedy for the illness with their special abilities, all while attempting to reveal the origins of the epidemic. However, the main characters in the game encounter more significant obstacles than the plague itself, needing to navigate deceitful NPCs, the deteriorating law and order in the increasingly sick town, and an upcoming military attack that could lead to the town’s demise if not prevented.
In 2019, the series was relaunched with Pathologic 2, focusing only on Artemy Burakh’s story, with the assurance that development for the other two playable characters was ongoing. Regrettably, issues related to Russia’s conflict in Ukraine (which the game’s creators have openly criticized) resulted in complications that caused a postponement in the launch of Danil Dankovsky’s narrative. Supporters of the series have eagerly awaited news on the upcoming installment of the Pathologic franchise for years, and at last, almost five years later, their patience has been rewarded.
